Nursing Services

MMHDS, Inc. offers Nursing and Case Management Services to all Service Recipients based on the needs identified in their Individual Support Plan.

Case Management Services are offered to all Service Recipients who are served in the Residential Program of MHDS, Inc. These Case Management services include monitoring of all of the Service Recipients medical and social needs. Each Residential Service Recipient is assigned to a Residential Team Leader or Case Manager, who assists the person in scheduling medical appointments, provides or arranges transportation to these medical appointments, oversees prescribed medications, and acts as a liaison between the Service Recipient and their family or other non-agency supports.

Direct Nursing Services are provided to any Service Recipient who physician has ordered a medical treatment that can only be provided by a licensed Nurse. MHDS, Inc. employees both Registered Nurses and Licensed Practical Nurses.

Other routine Social Services are provided to Service Recipients who do not receive Residential Services. These social services include assisting the Service Recipient and/or their family in keeping required documents updated, such as annual medical exams and financial eligibility paperwork. Each Service Recipient is assigned a Case Manager who will help keep the Service Recipient and/or their family informed of needed documents that are required for continuation in the program. These Social Services for Service Recipients who receive Day Services only do not routinely include scheduling and transportation to medical appointment.
