Our Day Services Program provides a variety of options to meet individual needs and preferences. We offer the opportunity for people to modify their day services as their interests and/or needs change over time through their Individual Support Plan (ISP).
Staff provides support with all aspects of job readiness including workplace expectations, training and skill development, resume development, interview techniques, job search, and on-the-job training. Employment specialists are available to assist in ensuring a successful employment result and will return to provide coaching daily or as needed.

HCBS includes services for individuals unable to take care of themselves without the assistance of others due to chronic illness, advanced age or cognitive impairment.
Currently, MHDS is approved to provide adult day care which provides supervised care and activities during the day at our facilities in Jackson, Brownsville and Bolivar.
Participants will be engaged in activities appropriate to their functioning level while their safety and comfort are continually monitored.

We provide community-based services to individuals who need social support or assistance with daily activities and who participate in services at one of our day sites. We offer a variety of activities within the day sites such as daily living skills training as well as leisure and recreational activities during the day. Day services are provided 6 hours a day with transportation provided. Individuals can choose a combination of community based and classroom type activities.

Transportation is provided to ensure a person supported can get to and from activities detailed in their Individual Support Plan (ISP).
Who Is Eligible?
Referral from the TN Home & Community Based Medicaid Waiver (HCBS). Individuals wishing to enroll in the HCBS Waiver must meet state requirements which are detailed at www.tn.gov/didd/topic/case-management or may be obtained from our office.
