ESSENTIAL JOB FUNCTIONS:

Provide on-site support and training with the Service Recipient employer specified job, interpersonal relationships with co-workers, and time and attendance processes.

Schedule and/or arrange for transportation for the Service Recipient to and from the place of employment.

Document Job Coach efforts in notes to indicate the amount of assistance the Service Recipient requires, their response to the job, and any barriers that are preventing total independence in the workplace.

Promote a positive image of the agency through the establishment of positive communications with the employer.

Meet at least one time per month with the assigned supervisor of the Service Recipient to communicate any needs or special considerations that will promote the intended independence of the Service Recipient at their job.

Coordinate trainings with the employer if necessary regarding special medical or behavioral concerns that may impact the Service Recipient in the workplace.

Implement the action steps of the Individual Support Plan related to Supported Employment and on-site work.

Develop and implement positive methods to assist the Service Recipient in reaching the goal of working independently, including fading techniques that encourages the Service Recipient to complete the job more independently each day.

Implement and document on specialized therapeutic plans as applicable for the Service Recipient.

Attend Circle of Support meetings to assist in the development of the Individual Support Plan.
