ESSENTIAL JOB FUNCTIONS:

Provide on-site support and training with the Service Recipient employer specified job, interpersonal relationships with co-workers, and time and attendance processes.

Schedule and/or arrange for transportation for the Service Recipient to and from the place of employment.

Document Job Coach efforts in notes to indicate the amount of assistance the Service Recipient requires, their response to the job, and any barriers that are preventing total independence in the workplace.

Promote a positive image of the agency through the establishment of positive communications with the employer.

Meet at least one time per month with the assigned supervisor of the Service Recipient to communicate any needs or special considerations that will promote the intended independence of the Service Recipient at their job.

Coordinate trainings with the employer if necessary regarding special medical or behavioral concerns that may impact the Service Recipient in the workplace.

Implement the action steps of the Individual Support Plan related to Supported Employment and on-site work.

Develop and implement positive methods to assist the Service Recipient in reaching the goal of working independently, including fading techniques that encourages the Service Recipient to complete the job more independently each day.

Implement and document on specialized therapeutic plans as applicable for the Service Recipient.

Attend Circle of Support meetings to assist in the development of the Individual Support Plan.

Hours:  8 pm – 8 am on Saturday and Sunday

Assigned Work Site:  2-3 person all male or female home located in Jackson and/or Madison County

Required Job Functions:

  • Assignment will be in supported living homes primarily and community locations as needed.
  • You must complete training on the unique personal needs of all individuals that you are assigned to work with.  No two persons supported require the exact same services or have the exact same needs or interests.  You must interact and respond to each person supported as an individual.
  • Make sure individual specific outcomes and action steps are followed and addressed.
  • Work direct care shifts, as scheduled and required.
  • Must provide quality services while promoting self-reliance and personal choice.
  • Responsible for the health and safety of the persons in their care.
  • In charge of the safe storage, handling and preparation of nutritious food in the homes.
  • Provide assistance to maintain the good health, hygiene, and personal needs of a person.
  • Complete and/or follow-up on the completion of timely documentation needed for incidents, behavior tracking data sheets, accident reports, etc.
  • Advocate on behalf of the person(s) you support, where appropriate.
  • Report medical and/or behavioral incidents as trained/instructed.
  • Perform routine housekeeping (dusting, vacuuming, mopping, laundry, sanitizing kitchen/bath, etc.)
  • Complete all assigned paperwork thoroughly and accurately with timely follow-up on corrections as needed.

Applicant must have 5 years Training experience with a 4 year college degree.

The Human Resources Trainer is responsible for the implementation of the required training curriculum as stated by DIDD, MCO’s and MHDS. The Human Resources Trainer will develop and implement a system for notification to all employees for trainings due and ensure communication across all departments in regards to training compliance

Conduct all trainings in compliance with required timeframes as indicated by DIDD, MCO’s and MHDS.

Develop classroom style curriculum for ECF/DIDD/MHDS training as required.

Be a liaison during Surveys with MCO/DIDDS.

Teach CPR/FA.

Track and ensure timely completion of other required trainings as indicated by DIDD/MCO’s and MHDS.

Assist in background checks on employees as required by DIDD/MCO’s..

Assist in new hire paperwork.

Learn how to utilize Relias and TIMAS for the ability to teach the training to others.

Receive and document all employee trainings in the TIMAS data base and ensure that all paperwork is filed/scanned in the employee training file in a timely manner.

Schedule and notify employees of trainings and ensure proper and timely notification to the employee’s supervisor.

Document late trainings and employee no-shows and notify the Human Resource Director and the employee’s supervisor of any deficiencies.

Assist with general Human Resource functions.

The LPN is responsible for direct support of the Service Recipient in the home and during community activities.  The goal of the LPN is to ensure that all assigned Service Recipients receive the necessary care to achieve a safe, meaningful, and positive home experience.  It is the responsibility of the LPN to develop and teach Service Recipients skills that will enhance their level of independence in the home and community setting.  The goal for all Service Recipients is to achieve total independence in their home and community environments. Ensure all ordered medications and treatments are provided to the Service Recipient according to the Plan of Care and document actions in the Nursing Progress Notes.
